Online system will see jump in UK adverse incident reports

Executive Summary

Data from early pilot programmes of the National Patient Safety Agency's on-line reporting system for all adverse incidents indicate that the system will increase significantly the numbers of reports received by the Medicines and Healthcare products Agency. The aim of the NPSA's system is to have a reporting system for logging all failures, mistakes, errors and near-misses across the health service in a non-blame culture, with the goal of ensuring that lessons are learned and spread throughout the health service. The results of the programme will be fed into the regulatory agency's adverse incident reporting system.
