Executive Summary
Mayo Clinic is conducting a year-long study to assess whether home monitoring of 200 seniors with chronic disease using GE Healthcare/Intel's Health Guide remote patient monitoring technology will reduce hospitalization and emergency visits. Patients will measure their vital signs and respond to disease-specific questions each day, and the data will be reviewed by a clinical care team working with their primary physician, with the aim of identifying early signs of clinical deterioration.
