Executive Summary
CMS says June 11 it is considering a request from the Society for Vascular Surgery that the agency use national society registries to collect data on carotid stenting - rather than a less robust CMS data submission system - as part of its ongoing coverage reconsideration for the procedure. SVS, which has run a carotid stent registry since 2005, says its registry and another sponsored by the American College of Cardiology (the NCDR CARE registry) will capture more detailed information to perform a risk-adjusted analysis of the procedure. SVS also asks that data reporting requirements be extended beyond initial hospitalization to at least 30 days and potentially 12 months post-procedure. CMS is considering expanding national coverage of carotid stenting at the request of ACC and three other professional societies (1"The Gray Sheet" Feb. 11, 2008, p. 6). Carotid stenting status quo
In a 1July 31 draft decision memo, CMS proposes to keep its current national coverage policy for carotid stenting, which covers stenting with embolic protection at a CMS-approved facility in high-risk patients with symptomatic carotid artery stenosis greater than 70%. The agency says "currently published data is not sufficient to expand coverage beyond the currently covered patient populations" and that it "believes in the importance of completing the ongoing randomized clinical trials and encourages future RCTs with medical therapy comparator arms." In December, a group of professional societies asked CMS to extend coverage to patients at high risk due to defined anatomic factors, and who have either symptomatic carotid artery stenosis of 50% or greater or asymptomatic carotid artery stenosis of 80% or greater.
